FAMILY CONNECTIONS INTAKE FORM

Thank you for your interest in the Family Connections™ program. Please read this important information and complete the Intake Form to be notified when program registration opens.

IMPORTANT: This form is NOT a registration form for a Family Connections™ group. We will email you when we can offer you space in an upcoming group. You will then need to complete a registration form and select your preferences for the program day and time. 

The Sashbear Foundation provides Family Connections throughout Canada in English and French. This form is for the English program only. If you want to take the program in French, please visit https://sashbear.org/fr/connexions-familiales/

Family Connections is an evidence-based program. It provides education, skills and support for people in a relationship with someone who has emotion dysregulation, BPD, or BPD traits. For more information, see our website: https://sashbear.org/en/family-connections

Family Connections is always provided free-of-charge to participants. Donations to Sashbear are welcome to help us continue to provide this program throughout Canada.
